Director (RN) Case Management

HCA Houston Healthcare Kingwood is a 439-bed acute care facility that has provided quality care to the Lake Houston area for over 25 years. It is home to the area’s only comprehensive cardiac, neuroscience and women services programs. Our Women & Children’s Center provides Level II and III neonatal intensive care units, high-risk obstetrics, a breast diagnostic center, a designated pediatric unit, and the area’s only pediatric emergency department.

HCA Houston Healthcare Kingwood is affiliated with HCA Houston Healthcare, part of the HCA Healthcare Gulf Coast Division. The division is a comprehensive network of hospitals, outpatient surgery centers, emergency centers and diagnostic imaging facilities in greater Houston, Corpus Christi and South Texas. Facilities include: 17 hospitals, nine ambulatory care centers, 13 off-campus emergency centers, and a regional transfer center.

What you will do in this role:

The Director of Case Management is responsible for the day to day operations of the department. The Director will develop, implement, and review the Utilization Management Plan annually. The Director is responsible for coordinating and facilitating multiple resources, internal and external, into a unique and single approach to care coordination for each patient assigned case management services. The Director will identify and implement processes that support and promote efficient patient throughput across the healthcare continuum resulting in optimal clinical outcomes, appropriate utilization of resources, accurate reimbursement and compliance with federal, state and payor regulations and guidelines. The Director will provide leadership, communication, and supervision to the department staff to ensure company and facility strategy is operationalized and adherence to applicable policies.

  • Collaborates with Director and/or Manager to assure adequate staffing for department.
  • Assumes responsibility to address issues of concern with staff members, including verbal and/or written counseling for poor performance and/or behavior.
  • Assists with operational aspects of the department such as monitoring of staff and leadership support to the Manager and Director.
  • Provides appropriate patient and/or family education or training based on assessed needs.
  • Demonstrates knowledge and skills necessary to assist/provide services to patients in relation to growth and development over the life span, including the ability to assist in the identification of requirements relative to age-specific and handicapped patient needs.
  • Accountable for reviewing, monitoring and reporting hospital utilization of resources through concurrent review according to pre-established guidelines and criteria and against established critical pathways.
  • Reviews and, if necessary, appeals Medicare/Medicaid and Managed Care denials.
  • Monitors for appropriateness and timeliness of services rendered.
  • Accountable for identifying other options for care when patient no longer requires acute hospitalization. Facilitates discharge planning utilizing social services, home health, and community resources.
  • Assists in coordination and evaluation of activities to assure hospital compliance with JCAHO requirements and other regulatory organizations.
  • Assists in preparation of case management reports to administration and other required committees, assist with budget and planning requirements.
  • Assists in development and implementation of a physician advisor program.
  • Assists in maintaining and updating methodology, tools, manuals and criteria as necessary and appropriate.

Qualifications

What qualifications you will need:

  • Bachelor of Science in Nursing
  • 5 years nursing case management experience required
  • 2 years Director or Supervisor experience preferred
  • Registration with the Board of Nurse Examiners for the State of Texas
  • Certification in Case Management, Nursing or Utilization Review preferred
