POSITION SUMMARY:

Under general supervision of the Director of Inpatient Rehabilitation and/or AVP of Rehabilitation Services, the PAI Care Coordinator is responsible for PPS/PAI data and transmitting that data to CMS. This includes assisting/assuring all disciplines maintain current educational requirements to maintain regulatory compliance.

PAI Coordinator is also responsible for patient care in accordance with established policies and procedures of Medical City Healthcare.

In collaboration with the medical staff, patients, patient’s family and other members of the health care team, the PAI Coordinator is responsible for utilizing a holistic and interdisciplinary approach to coordinate the care of all patients.

In addition, the Coordinator may also be responsible for assessing nursing needs of patients (and in collaboration with Rehabilitation Services), planning interventions and evaluating outcomes of care.

The PAI Coordinator may also provide direct care within their scope and practice and delegate/collaborate care with other disciplines and non-licensed staff as it relates to the outcome and care of the inpatient rehabilitation patient.

Graduate of an accredited school of nursing (minimum BSN), physical therapy, speech therapy or occupational therapy.

If hired after 6/1/12, BSN required within 24 months of hire into RN position.

Clinical experience in the care and management of the Inpatient Rehabilitation patient.

Minimum of 3 years recent clinical experience in inpatient rehab preferred and 1 year experience coordinating Prospective Payment System (PPS) and the Inpatient Rehabilitation Facility-Patient Assessment Instrument (IRF-PAI)

Must have well established leadership skills.

Must possess program development skills & have experience in infrastructure development.CERTIFICATION/LICENSURE:

Current Texas licensure/certification – BS minimum in current field of study (nursing, physical therapy, Speech therapy or occupational therapy)

BCLS certification (AHA)

Pass the licensed personnel FIM exam using the UDSMR Online Credentialing System within 60 days of hire and again as necessary to maintain the facilities UDS-PRO credential.

Advanced certification preferred
