Position Summary:This position will play an important role in supporting the current and future systems change efforts within the Health Equity Collective. The Health Equity Collective was developed to serve as a long-term, durable driver for improving health and health equity through collaboratively improving social determinants of health systems. This ecosystem approach in order to improve health equity includes more than 180 organizations with a singular mission of establishing a sustainable, data-driven, human-centered ecosystem of care that equitably addresses social determinants of health among residents of the Greater Houston area. UTHealth serves as the backbone organization, anchoring this collective impact effort.Manages and facilitates collaborative processes for community engagement, community health worker (CHW) initiatives, and care coordination efforts.Serves as CHW network lead, cultivate community voice engagement and strategy, and health equity ambassador.Works with existing community voice engagement, CHW, and care coordination efforts to identify opportunity communities for increased capacity and assists project leadership in all aspects of the project. Supports in the collaborative development and refinement of project strategy and operationalizing the strategic plan, processes and protocols. Leads training of staff, development of tracking mechanisms for the project success metrics, assist with coordination of all aspects of the network mapping, alignment, and health equity expansion, coordinating training and capacity building and developing related strategic plans, performing evaluation procedures, and keeping project leadership informed about the measurement progress.Supports community voice workgroup and member organizations in further developing community voice engagement strategies.The ability to travel to various locations throughout Harris County is expected.Position Key Accountabilities:

Oversees all aspects of the program activities. Sets deadlines, assigns responsibilities, and monitors project progress. Develops, implements, and manages policies and procedures.

May manage/maintain annual budget, negotiate service contracts, report on progress and expenditures.

Informs upper management of key activities and makes periodic progress reports.

Works closely with internal departments regarding strategy and collaborative opportunities.

Prepares progress reports and supports in writing of grant proposals to both private foundations and governmental agencies; works closely with the Office of Sponsored Projects on the submission of grants.

Interacts with representatives from state, federal, and private agencies. Collaborates with local organizations and community leaders on event and partnership activities and opportunities.

Leads the communication with the project team to help coordinate implementation and evaluation and participate in report writing, story gathering, web content, newsletters, and publications, in conjunction with Public Affairs.

Communicates with community and professionals to share objectives and outcomes. Prepares/makes oral presentations at professional meetings.

Manages and facilitates collaboration between existing CHW efforts, community leaders, and community voice engagement from diverse communities.

Develops and implements strategic plans for CHW network development, operationalizing equity, and tracking mechanisms for the project metrics of success.

Coordinates of all aspects of the CHW network mapping, alignment, with focus on expanding capacity of CHWs from and in BIPOC communities, in collaboration with Health Equity Collective working groups and member organizations.

Manages alignment and enhancements of training and capacity building for the CHW network, including workforce development and sustainability strategies.

Collects, evaluates, and interprets project data. Maintains confidential records of collected data, ensures accuracy and integrity of data. Data may include patient data and departmental programmatic data.

Actively participates in related Health Equity Collective efforts and facilitates linking of CHW strategies to broader collective strategies.

May manage Human Resources activities for direct report in regards to: recruiting and selection, hiring and terminating, training and development, performance evaluations, and salary planning.

Performs other duties as assigned.

Preferred Certification/Skills:

Excellent communication skills both written and verbal.

Working knowledge of MS Office.

Google Workspace

Willingness to work some evenings and weekends.

Able to travel to offsite locations independently.

Minimum Education:Bachelor’s degree or relevant work experience in lieu of education.Preferred:

Familiarity with the local community

Experience working with diverse stakeholders

Understanding of project management processes

Community Health Worker experience and/or certification

Bi-lingual English and Spanish fluency
