Theyare responsible forconducting educational groups, performing strengths and needs assessments, andmaintainingcommunication links between the treatment team and families, guardians, and community providers. Additionally, they coordinate all aspects of discharges to ensure patients receive theappropriate levelof care and support.Key Responsibilities
Providescomprehensive therapeutic services and interventions for barriers to discharge.
Coordinates all aspects of discharge planning and documentation, and advocates for patients within the scope of clinical training and professional standards.
Enhances patientcapacityto address their own needs and uses patient contact as opportunities for therapeutic intervention.
Cooperates with colleagues and seeks advice and counsel when in the best interests of patients.
Attends weekly department meetings and actively contributes to problem-solving and idea sharing within the department.
Qualifications & RequirementsEducation
Bachelor's degree from an accredited college or university required.
Experience
Minimum of two (2) years practical experience working in social services, human services related field and the chronically mentally ill population.
