Job Description:The Advance Practice Provider (APP) is responsible for providing direct patient care in short term rehab and skilled nursing settings in the Salt Lake Valley area.The APP participates as a leader of the Post-Acute Care Team (PACT) for designated Skilled Nursing Facilities (SNFs). The purpose of the PACT team is to improve patient outcomes and reduce the overall cost of care. The APP will coordinate care with discharging hospitalists, primary and specialty care physicians, hospital and clinic based care managers and SNF provider and social work staff. The APP will work with SNF staff in developing a patient-specific post-SNF care plan, reconciling medications at discharge, and communicating with clinic based care manger to assure smooth post-SNF transition.The ideal candidate will have a background in post acute care, and be able to demonstrate flexibility, problem-solving skills, ability to work independently and as part of a team.Job EssentialsAdvanced Practice Providers (APPs) are accountable for the provision of patient care at the top of licensure, within their scope of practice. Licensed, credentialed and privileged to perform such duties as:Maintenance and promotion of health and prevention of disease;

Assessment, diagnosis, treatment, correction, and consultation;

Procedures as outlined by specialty, and referral for health problems;

Education of the patient, patient's family, clinical staff, and others;

Prescription or administration of prescription drugs or devices in accordance with a consultation and referral plan;

Consultation with other healthcare providers;

Appropriate and timely documentation of clinical care;

Continually learning and developing for optimal patient care and outcomes.

Advanced Practice Providers may also participate in research, utilize evidence-based practice to improve quality patient care, protocol development, care process model development, and best practice standards for assigned patient population throughout our organization.Minimum QualificationsCurrent state license as an Advance Practice Registered Nurse (APRN) or Physician Assistant (PA).and -

Certification as a Nurse Practitioner or Physician Assistant.and -

Eligible for State Controlled Substance License II through V.and -

Eligible for Federal DEA Certificate II through V.and -

BLS Certification for Healthcare Providers.and -

Valid drivers licenseand -

Ability to commute to the office at least twice daily to electronically transfer patient files or have other access to the Internet for electronically transferring patient files twice a dayand -

Insured and reliable transportation and an acceptable driving record according to Intermountain policyPhysical Requirements:Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.and -

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.and -

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.and -

Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.and -

Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).and -
