Job Description:The Homecare RN Case Manager serves as a key member of the interdisciplinary team and actively manages and directs resource utilization to achieve the highest quality outcomes during a patient care experience. The incumbent oversees the effective coordination of services and manages issues in the following main areas: admission and discharge, team conference and interdisciplinary plan of care communication, patient and family education/relations, payor relations, and coordination of care for all disciplines and total fiscal management. The incumbent performs ongoing utilization and quality reviews and acts as a liaison between the payor and patient while assuring that cost-effective treatment is provided by the team. The incumbent performs RN Shared Leadership duties by supporting the fundamentals of safety, quality, experience, and stewardship by providing clinical expertise and leadership to their department. The Homecare RN Case Manager has significant leadership responsibilities (e.g., leading change, scheduling, maintenance of staff competencies, new employee selection/orientation, constructive discipline, shared leadership council representation, caregiver check-ins). Develops relationships and collaborates with physicians, facilities, and teams to promote process integration, seamless transitions from one program to another, continuity of care, and to avoid duplicative care management services and processes.2. Manages the care for clinically complex patients admitted to homecare. Meets with the patient or family in a timely manner and conducts an initial assessment using the appropriate federal assessment.3. Develops/Reviews a patient-centered plan of care, involving the patient or family or caregiver or significant others in the process to develop goals/outcomes.4. Advocates on behalf of the patient by communicating and collaborating with healthcare providers, payers, physicians, and community-based services, where appropriate, to establish an appropriate and integrated care plan for each patient.5. Facilitates transitions of care from one healthcare setting to another.6. Functions as the team leader to ensure effective day-to-day operations and problem solving, for the Home Health or Hospice team. Oversees care team and coordinates patient activities to ensure quality and cost-effective care.Minimum QualificationsBachelor's degree in Nursing (BSN) from an accredited institutionCurrent RN license in state of practiceCurrent driver’s license, insured and reliable transportation and an acceptable driving recordCurrent Hospice Certification or Oasis Certification designation must be achieved within 12 months of hire/transition into this position

Current BLS Certification for healthcare providersTwo years of clinical nursing experienceOne year of experience working in Home Health or Hospice setting.Ability to communicate effectively both verbally and in writingPreferred QualificationsThree years of experience working as a case/care manager in a home health or hospice settingExperience working with third party payersKnowledge of home- based health services and resources#LI-RC1

Physical Requirements:Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, and identify equipment and supplies.and -

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.and -

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, and driving a vehicle.and -

Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.and -

Frequently walks and assists with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling portable equipment, including heavy items. Often required to navigate crowded and busy rooms (e.g., full of furniture, equipment, power cords on the floor). Need to ascend and descend stairs or uneven surface to access patients.and -
