Under the supervision of a registered nurse, the licensedvocational/practical nurse medical surgical assumes responsibility andaccountability for the application of the nursing process and the delivery ofpatient care. Consistently performsaccording to nursing standards and is accountable in managing patient care andassisting others in the management of patient care. Essential Duties and Responsibilities:

Under the direct supervision of a Registered Nurse: utilizes the nursing process to assess, plan,implement and evaluate patient care

Is aware of physiologic and psychosocial changes in thepatient’s condition

Collects, analyzes, and interprets data and information fromhealth care members and documents actual and/or potential nursing diagnoses

Documents the patient’s plan of care using identifiednursing diagnoses, expected patient outcomes, and selected nursinginterventions

Identifies and prioritizes actions based upon patient carerequirements

Performs interventions according to identified priorities,plan of care, and the hospital policies and patient care outcome standard

Provides a safe, comfortable and therapeutic patientenvironment

Documents patient’s response to nursing and otherinterventions

Uses clinical judgment in evaluation activities to meetpatient care needs of and assigned unit/floor including establishing priorities

Monitors the safety and effectiveness of the environment andequipment Performs other duties as assigned/necessary

Minimum Requirements:

Current Licensed Practical/Vocational Nurse License in statepracticing

Complies with all relevant professional standards ofpractice

Participation and completion of Amergis' Competency programwhen applicable

Current CPR if applicable

TB questionnaire, PPD or chest x-ray if applicable

Current Health certificate (per contract or stateregulation)

Must meet all federal, state and local requirements

Successful completion of new hire training as applicable tojob site

Understand patient confidentiality and HIPAArequirements

Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required

Computer proficiency required

Must be at least 18 years of age
