Job Summary and Qualifications

Seeking an experienced Case Manager/Social Worker to join our dynamic team of healthcare professionals. In this role you will collaborate with the healthcare team to provide initial and ongoing psychosocial assessments for patients and families having complex psychosocial needs and/or medical complex diagnosis. You will develop treatment goals and plans to resolve psychosocial concerns.

• You will perform admission and concurrent review for Medicare payers and maintain legible documentation as direct by the Utilization Management Review.

• You will educate patients and families to the patient’s needs post discharge and inform them of any alternatives to meet those needs.

• You will arrange transfer and referral for any patients who require extended care post discharge to post-acute care facilities.
• You will collaborate with community agencies to provide services to assist with housing, financial, transportation, home health, and any other needs of patient or patient’s family upon discharge.
• You will document all assessments, treatment goals, interventions, outcomes, and discharge plans in the patients’ medical record.
• You will provide crisis and supportive counseling and any needed education to enhance the patients problem solving and coping skills.
• Your will communicate effectively with patients, teammates, and physicians to ensure continuity of care

What qualifications you will need:

• At least one year previous paid relevant employment or a field placement in a health care setting.
• Two years previous paid relevant employment or a field placement in a health care setting is preferred.
• Bachelor Degree in Social Work or Human Services field.
• Master of Social Work Degree or Human Services field is preferred.
