Duties include assessing patients interest in Continue Care Pharmacy Program, supporting a medication management program, and conducting in-home patient enrollment visits. ResponsibilitiesContacts patients who have been deemed eligible for the Continue Care Pharmacy Program
Conducts Branch Director meetings every two weeks for feedback and program oversight.
Tracking and trending of program metrics
Attending staff meetings/case conferences as needed
Program resource for field clinicians
Liaison between branch and program leadership
Conducts in-home patient enrollment visits
Provides additional home visits to assist patient with medication synchronization and use of the Continue Care Medication Packs
QualificationsLPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice OR
Nationally certified Pharmacy technician in the state of practice OR
Licensed Social worker with a degree from an accredited college with current unrestricted registration and license in the state of practice
Valid driver’s license, acceptable driving record and proof of car insurance
1+ years of medical related experience preferred
Home health care experience preferred but not necessary
Able to bend, stoop, push, pull, sit, and walk for periods of time
