Job Title: Care Navigator

Team: Housing

Reports To: Director of Housing

Hours: 40 hours per week

Compensation: Starting pay $24-26/hr depending on experience

HopeSource seeks to utilize a Whole Person Care approach to promote individual wellness by observing, discussing, and addressing all dimensions of a clients life by lowering barriers to resources that equip individuals with what they need to achieve their goals.

The Care Navigator helps clients get the support they need to access a wide range of resources. Navigators provide personalized support and guidance to access HopeSource programs as well as provide a direct connection to vetted community agencies and resources that support the needs of the individual. Supports include but are not limited to addressing economic barriers, providing basic needs (including, but not limited to, housing, food, transportation, utility payments, and material goods), and other supportive services such as workforce development, health and mental health care, recovery, and life skills education.

Navigators are skilled, effective communicators who have a problem solving mindset and are responsible for ongoing community resource knowledge. Navigators collaborate extensively within the community to overcome logistical barriers and increase care quality, improve care continuity, and improve the customer experience.

Responsibilities:

Provide clients with the best experience possible in receiving whole person care through a person centered, trauma informed approach.

Provide a single point of contact for clients within HopeSource to address a wide variety of needs.

Provide connections to other systems of care while ensuring continuity between internal and external referrals and services.

Streamline client access to all services ensuring excellent and uncomplicated quality of care to the greatest extent possible.

Advocate and refer for services, including financial assistance, legal aid, housing, job placement, education, primary healthcare, mental health, substance treatment and other client-identified needs.

Possess a broad understanding of all available programs, services, and community providers.

Engage in client-focused case conferencing with partner agencies to ensure clients are receiving assistance to access the services they need for long term stability

Support clients as they navigate complicated systems and remove barriers where possible.

Advocate for clients until they reach self-sufficiency.

Other duties as assigned.

Qualified Candidate will possess the following skills:

Effectively and tactfully handling difficult or sensitive issues using tact and diplomacy, demonstrating genuine concern for others.

Effectively communicates, building rapport and relating well to individuals with diverse backgrounds regardless of differences.

Handles any situation gracefully by using non-verbal communication, in-depth questioning and active listening skills in order to recognize other perspectives.

Collaborates with others to meet objectives, sharing responsibility with team members for successes and failures, recognizing and appreciating the contributions of team members.

Competent computer skills with proficiency in Microsoft Office products and ability to learn new technical skills

Ability to demonstrate flexibility and to adapt when faced with internal or external barriers, or when faced with differing points of view

Ability to anticipate next steps, be proactive and collaborate with coworkers and stakeholders

Ability to use integrated technology platforms and virtual care coordination tools

Ability to use good judgment and problem-solving skills, able to effectively respond to difficult situations and resolve conflicts

Ability to work independently in a fast-paced environment

Ability to organize and prioritize workload; effective time and task management skills in achieving program initiatives and priorities

Ability to successfully complete tasks with little or no supervision.

Ability to anticipate patient and team member needs

Bi-lingual English/Spanish preferred
