Description:Providence is calling a Home-Based Acute Care Physical Therapist (Providence At Home Olympia, Days, On Call) to PSCS Divisional Services in the Olympia, WA area. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all - especially those who are poor and vulnerable.The Home-Based Acute Care Physical Therapist is responsible for therapy evaluations, treatment planning, implementation, and discharge planning for inpatient hospital at home patients in the home setting in accordance with established policies while maintaining the highest degree of quality patient care. As a member of the multi-disciplinary team the therapist assumes responsibility for communication of patient-related issues with appropriate team members or other staff and participates in collection and evaluation of data necessary to assess appropriateness of care at home. The Physical Therapist may also be assigned to orient staff and students. The Physical Therapist works with the patient and family/caregivers in the home setting without direct supervision.Apply today! Qualifications:Required qualifications for this position include:Education sufficient for licensure

1 year of therapy experience with adult geriatric and/or individuals with physical disabilities

Current Washington State Physical Therapist License

Valid Driver’s license

Current CPR certification required within 30 days of hire.

Requires access to daily use of an insured reliable vehicle or an alternate ability to travel to sites as required.

Preferred qualifications for the position include:1 year of therapy experience in home care or in related rehabilitation setting

Experience with the geriatric population
