Position Details:13 weeks, Full-time - 40 hours, 8a-5p, M-F. Weekends: 1/3 weekends in rotation with staff - Experience: in-Home Care, Oncology, Hospice. Social worker will float between client’s territories as well. Social Worker must have a Master’s Degree and be licensed in WA State. Master's Level MSW, MHC or MFT - mental health. Responsibilities for this Case Manager opportunity include, but are not limited to:Responsible for the coordination of health care decisions by using a systematic approach to ensure treatment plans that improve quality and outcomes

Responsible for the promotion of cost-effective care within allotted time frame

Assure payments of hospital-based services meet patient-related utilization of management criteria

Implementation of safe and appropriate discharge plans

Asses the social work needs of the patient and provide intervention as part of the discharge planning process

Primary functions include: Clinical Interventions/Discharge, Care Planning Management, Fiscal Manage and Payer/Referral Management

JOB REQUIREMENTS FOR THE LCSW LICENSED CLINICAL SOCIAL WORKER:Master’s Degree in Social Work

Current state licensure as a Licensed Social Worker

2 years of recent experience in social work, case management or related area

Must be eligible to work in the United States
