The Home Health Registered Nurse is responsible forproviding and documenting skilled one-on-one nursing care in accordance withthe physician-ordered individualized patient care plan. This care is provided mostly in the patient’shome or a residential setting but can transfer outside the home duringappointments and other activities the nurse may accompany the patient to. Essential Duties and Responsibilities:
Completes an initial assessment of patient and family todetermine home care needs when applicable
Provides a complete physical assessment and history ofcurrent and previous illness(s)
Regularly re-evaluates patient nursing needs
Initiates the plan of care when directed and makes necessaryrevisions as patient status changes
Uses health assessment data to determine nursing diagnosis
Establishes goals based on nursing diagnosis andincorporates therapeutic, preventative, and rehabilitative nursing actions
Includes the patient and the family in the planning andexecution of the treatment plan and goals
Initiates appropriate preventative and rehabilitativenursing procedures
Administers medications and treatments as prescribed by thephysician
Provides healthcare instructions and education to thepatient as appropriate per assessment and plan of care
Identifies discharge planning needs when applicable
Adheres to home health agency policy and protocol
Attends case conference, team meetings, and educationin-services as required
Minimum Requirements:
Graduate of an accredited school of nursing
Currently licensed as a Registered Nurse in the state inwhich the RN will practice
Complies with all relevant professional standards ofpractice
Participation and completion of Amergis' Competency programwhen applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or stateregulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable tojob site
Understand patient confidentiality and HIPAArequirements
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
Computer proficiency required
Must be at least 18 years of age
