Chemical Dependency Professionals SUDP are responsible for facilitating problem solving & educational groups and individual session, and Morning Gathering. Additionally, the Chemical Dependency Professional must possess skill in crisis interventions and patient motivational skills to engage, as well as patient discharge confrontations when necessary.ResponsibilitiesProvides role-modeling behaviorResponsible for intakes, assessments, treatment plan development, file reviews, progress notes, aftercare plans and discharge summaries.Responsible for facilitating problem solving & educational groups and individual session, and Morning GatheringMust engage in crisis interventions and patient motivational skills, as well as patient discharge confrontations when necessaryResponsible for timely and proficient case management and documentation to include ensuring all necessary paperwork is sent in a timely manner for admission and discharge.This is a full time position that will work Monday-Friday 8am-4:30pm.
