3.Physical Damage provided by Lease

4. Occupational Insurance $45.00 per week

OPTION TWO:

Commission 12%

Occupational Insurance for owner $45.00 per week

Auto Liability and Cargo $280.00 per week

Logbook and Camera Service Fee $70.00

Trailer Rent $250.00 per week

Physical Damage truck/trailer per week $70.00/$50.00
