Program Implementation Coordinator at SomatusRemoteSummary:The Program Implementation Coordinator, reporting to the Senior Director, Program Development, will assist in coordinating tasks required for new and existing implementations. This person will be responsible for collaborating with multiple departments to ensure stakeholders are aligned and processes are streamlined.Essential Duties and Responsibilities:Developing, maintaining, and managing detailed project plans, task due date, and major milestone timelines for assigned projects within Smartsheet.

Point of contact for project status.

Responsible for maintaining communication with stakeholders across multiple departments.

Develop and communicate clear objectives and delivery expectations, as defined during the project kick-off and discovery process.

Coordinate with program managers (RN, NP, SW, RD, PHA disciplines) to intercept and identify ownership of projects among our operations, clinical program, and product departments.

Work alongside clinical and training directors to establish roadmaps for projects and determine staff task assignments.

Manage all assigned projects to completion; ensuring on-time completion of deliverables in conjunction with program and training directors.

Facilitate and lead project meetings, documenting summaries and communicating next steps.

Collect and analyze data regarding effectiveness and outcomes of projects completed by the program and training departments.

Ensure (in collaboration with program and training directors) that deliverables meet company policies and procedures, NCQA, MOC, CMS, and SLA requirements per our contracts and accreditations.

Two (2) years healthcare experience with population health, value-based care, and/or care management.

Project management experience.

Knowledge, Skills, and Abilities:Extensive knowledge and proficiency with all Microsoft tools including PowerPoint, Excel, Outlook, etc.

Knowledgeable in use of Smartsheet or other project planning platform(s).

Ability to manage multiple, competing priorities with minimal oversight.

Ability to handle ambiguity and desire to work in a fast-paced start-up environment.

Acute attention to detail.

Excellent written, verbal and organizational skills.

Ability to build and maintain collaborative relationships with key members of other departments within the organization.

Physical RequirementsThis job operates in a professional setting. While performing the duties of this job, the employee is regularly required to sit or stand for extended periods of time. Normal manual dexterity is required.

Normal speaking and hearing abilities to interact with others in an office environment, over telephone or other video conferencing platform.

The employee is occasionally required to stand; walk; and reach with hands and arms and continuously repeat the same hand, arm finger motion many times as in typing.
